Or play the games in a place where they will sell out aka not Toronto or Montreal. Or maybe they just really don't care about the optics of the matches.

Bang on! Like why doesn't the CSA ever play international matches in cities like Halifax? The German FA is already planning to host its next UEFA Nations League matches in smaller venues, because the next opponents that relegated Germany (don't you just love that) will be playing in League B will draw significantly smaller crowds then playing against the likes of the Netherlands and France.

I think the only optics that the CSA really cares about are playing their matches in stadiums which have wrap-around-the-pitch  ticker boards with advertising, because they make way more $$$ off that sort of thing than they do off of ticket sales. Personally I found it very refreshing to see the absence of such annoying advertising in the Saint Kitts match. That looked much more like a real soccer game.
